Declaration

The material I am submitting is my own photography/videography. I give permission for the photographs and/or videos in which I, and/or my child/ward, appear, to be used by Dream Blue for publications and promotional activities.

 

I also grant permission to Dream Blue to publish my first name, last name initial, state, country and quoted testimonial (in whole or in part). In particular, I note that these publications may include use in both print and electronic media, including the Dream Blue website and official social media accounts.

 

I have read and understand this notice, and consent to the collection, use and disclosure of my submitted images, videos, testimonial and information as outlined. I also acknowledge that I will not be owed any payment, remuneration or royalties from Dream Blue for using the photographs, videos and/or testimonial.
